  1. Upon referral, patients are assigned to our dedicated Case Managers and Clinical Coordinators.
  2. Intake Coordinators contact referral source and patient upon receipt of referral. After the initial contact, the intake department will check the patient’s eligibility to confirm the assigned payor.
  3. Miracle coordinates transition of care from the hospital or SNF discharge to the home. MHHC staff works hand in hand with the discharge planners to coordinate any DME/wound supplies and IV infusion that is ordered for the patient upon discharge.
  4. Our Case Managers and Clinical Coordinators will provide a follow up call after the **Start of Care** visit to welcome the patients to MHHCI and address any further questions or concerns that patient or family may have.

All services will be provided by licensed qualified professionals at Miracle Home Health Care, Inc.
